COVID-19 Vaccination Clinics for Students age 5-11

Fayetteville Public Schools, in cooperation with Medical Arts Pharmacy, will host COVID-19 vaccination clinics for our elementary and middle school students (ages 5 to 11) during the week of November 15-19. The clinics will be held at your child’s school during the school day. The list of clinic locations and dates is provided below:

  • Asbell Elementary: November 19
  • Butterfield Trail Elementary: November 18
  • Fayetteville Virtual Academy:  November 18 
  • Happy Hollow Elementary: November 18
  • Holcomb Elementary: November 15
  • Leverett Elementary: November 17
  • Owl Creek Elementary & Middle School: November 19
  • Root Elementary: November 15
  • Vandergriff Elementary: November 16
  • Washington Elementary: November 17
  • Holt Middle School: November 17
  • McNair Middle School: November 16

Students will receive their first dose of the Pfizer vaccine at these clinics. The second dose will be given at clinics that will also be held at your child’s school during the week of December 13-17. You will receive more information about the second dose clinics in the coming days.

To receive the vaccine, two forms must be turned in by November 12, 2021:

  1. CONSENT FORM: Please download, print, and sign the consent form at this link
  2. FERPA FORM: Students are also required to bring a FERPA consent form signed by a parent/guardian. It can be found at this link

Printed copies of both forms are also available in the school offices and will be sent home in folders next week. Parents, if available, please send a copy of your child’s health insurance information card (front and back) to vaccine clinics. Health insurance is NOT required to receive the vaccinations, and there is no charge to families for the vaccine.  IF YOU COMPLETE THE FORMS IN THE SCHOOL FOLDER AND RETURN THEM TO THE SCHOOL BEFORE NOVEMBER 12, YOU DO NOT HAVE TO COMPLETE THE FORMS LINKED IN THIS EMAIL. 

Please note:  parents may not attend the FPS COVID-19 vaccination clinics, these clinics will be held following the same protocol as district flu clinics.  If you wish to be present with your child for their COVID-19 vaccine, parents may opt to visit one of many local clinics and pharmacies that are providing the vaccine.
